Christopher R. Brigham, MD, the President of Brigham and Associates, Inc., is committed to working with others in the application of evidence-based medicine and best practices to promote function and avoid needless disability. He received his MD from Washington University School of Medicine and is board-certified in preventive and occupational medicine. He is the Editor of the AMA Guides Newsletter, has published over two hundred and fifty peer-reviewed articles, and edited and co-authored several previous books including the AMA Guides to the Evaluation of Permanent Impairment, Sixth Edition. He is passionate about creating change that will empower individuals to live joyful, productive lives.
Henry Bennett is currently President of Independent Resources, a publishing consultancy. Previously he was Director of Kamehameha Schools Press and Editor/Manager of Bishop Museum Press and began his book-publishing career with the University of California Press. He has over three decades of experience in writing, re-writing, editing, and publishing, providing support to writers/authors and commercial, institutional, and self publishers. He is a multiple past president of the Hawai‘i Book Publishers Association and lives in Honolulu.
